Registration for the event is not required, but recommended to save time at the event. Paper copies or digital records of previous vaccinations and health insurance information is helpful, but not required. View the flyer for the Student Vaccination Event.

This event occurs during the CCES Open House, so please plan accordingly.

Vaccinations offered: DTaP, Hepatitis B, Meningococcal, MMR, Polio, Tdap, and Varicella

Vaccinations offered (recommended, not required for K-12 students): Hepatitis A, Hib and Pneumococcal

State Law: According to the Minnesota Statute 121A.15, students enrolling in a school district must have the day, month, and year of all required immunizations on file at the school district they attend.

If you are unsure if your child has these immunizations, please contact your child’s health care provider or the Minnesota Immunization Information Connection (MIIC) at 651-201-3980.

If you are concerned about costs or need additional resources, contact your healthcare provider or your County Public Health Office for vaccination resources.

Updated forms: After completing immunizations, provide the health office at your child’s school with updated immunization records as a fax, screenshot, email, drop them off at the school main office or health office during school hours.

Exemption form: A Medical and/or Non-Medical Exemption form may be completed if you choose not to have your child immunized. Medical and/or Non-Medical Exemption forms must be notarized. A notary is available at your child’s school during school hours or at Open House events. Please call the main office to check availability.
